Records indicate the facility offered 58 beds, which is 18 fewer than the Coconino County average for that quarter. On average, 40.6 residents received care at the facility during this time frame.
The Peaks Health & Rehabilitation earned an overall 2 out of 5 rating from the CMS, trailing Arizona’s 3.4 nursing home average.
Haven of Sedona was the largest nursing home in Coconino County for the quarter, providing 112 beds. Haven of Flagstaff ranked second with 83 beds.
Through the first quarter of 2026, The Peaks Health & Rehabilitation did not report any fines or penalties.

U.S. Census Bureau data show the nation’s older adult population is rising while younger cohorts decline. From 2020 to 2024, Americans age 65 and up increased by 13%, as the population under 18 dipped by 1.7%. Over the past century, the country’s older adult population has expanded significantly—from 4.9 million in 1920 to more than 61 million in 2024.
A 2021 analysis found nearly half this demographic, about 28 million people, will likely require long-term services and supports. The report further says over a third might ultimately need nursing home care.
“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
