Flagstaff Unified School District recently issued the following announcement.
In partnership with Premier Medical Group, free COVID-19 follow up vaccines will be available for individuals ages 5 and up from 3:00 p.m. to 6:00 p.m. on Thursday, February 24, 2022, at Summit High School. The event will be in the gymnasium just inside the main entrance at 4000 N. Cummings St., Flagstaff, AZ 86004.
Pfizer vaccines will be available for individuals ages 5 and older, as well as Pfizer boosters for 12-17 year olds. Any individuals above the age of 17 seeking a second dose or booster of any of the vaccines (Pfizer, Moderna, or Johnson/Johnson) must bring their vaccination card with them to the Summit High School site. All vaccines will be available to qualifying individuals.
The timeframe of 3:00 p.m. to 6:00 p.m. will be open to the public. This is a first-come, first-served event and is walk-up only. No appointments will be offered.
Parental consent is required for children under 18 to receive the vaccine. Parents or guardians that do not wish to attend the event with their children should print and fill out the under 18 consent form COVID-19 Vaccine Consent Form completely in advance. Children under 18 will need to present the completed consent form to vaccine providers at the event. Parents or guardians who choose to be present with their children will also be able to fill out the consent form on site.
