Free COVID-19 Vaccinations Available at Summit High School on Thursday, February 3

Flagstaff Unified School District recently issued the following announcement.

In partnership with Premier Medical Group, free COVID-19 vaccinations will be available for individuals ages 5 and up from 3:00 p.m. to 6:00 p.m. on Thursday, February 3, 2022, at Summit High School. The event will be in the gymnasium just inside the main entrance at 4000 N. Cummings St., Flagstaff, AZ 86004.

Pfizer vaccines will be available for individuals ages 5 and older, as well as Pfizer boosters for 12-17 year olds. Any individuals above the age of 17 seeking a second dose or booster of any of the vaccines (Pfizer, Moderna, or Johnson/Johnson) must bring their vaccination card with them to the Summit High School site. All vaccines will be available to qualifying individuals. 

The timeframe of 3:00 p.m. to 6:00 p.m. will be open to the public. This is a first-come, first- served event and is walk-up only. No appointments will be offered. 

Parental consent is required for children under 18 to receive the vaccine. Parents or guardians that do not wish to attend the event with their children should print and fill out the under 18 consent form COVID-19 Vaccine Consent Form completely in advance. Children under 18 will need to present the completed consent form to vaccine providers at the event. Consent forms will also be available to fill out onsite.

Face masks are required for all individuals at the vaccination site.
