During the first quarter of 2026, Aspire Transitional Care was owned by Flagstaff Transitional Care, LLC and The Goodman Family Operating Foundation, data from the Centers for Medicare and Medicaid Services (CMS) shows.
The facility operates as a for-profit nursing home.
That quarter, Aspire Transitional Care earned an overall CMS rating of two, below Arizona’s average nursing home rating of 3.4. CMS rates facilities on key areas including health inspections, staffing, and resident care quality.
The nursing home served an average of 40.7 residents per day in the first quarter of 2026, with a capacity of 50 beds. Aspire Transitional Care is not hospital-affiliated.
No fines or penalties were reported for Aspire Transitional Care during that period.
Coconino County had three for-profit nursing homes in total.
U.S. Census Bureau estimates indicate the number of older adults in the country continues to rise, while the younger population declines. Between 2020 and 2024, Americans age 65 and above grew by 13%, compared to a 1.7% decrease for those under 18. Over the last century, the older adult population expanded significantly, increasing from 4.9 million in 1920 to over 61 million by 2024.
A study published in 2021 estimates that nearly half of older adults—around 28 million—may eventually need long-term services and supports, with more than a third expected to require nursing home care at some point.
“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
